What to do when someone you love dies

The death of a loved one can be devastating. We have created the following list of information required by the registrar, to help you in your time of need:

• Full name of the deceased person (Last address)
  
  
• * Date of birth
• * Place of birth
• * Date of death
• * Place of death
• * Marriage details (place, date and to whom married)
• * Children’s names and ages
• * Parents names (include mother’s maiden name)
• * Parents occupations (current or former)

We will complete the following statutory forms with you:

* Death Registration form
* Application for a Death Certificate
* Application for burial or cremation
